Job Responsibility Assists in identifying and using community resources for patients and/or their families including visits to patient's homes.
Assists in establishing liaison and communications with other agencies including visits to agencies.
Maintains a variety of records and reports relating to social work activity Performs related duties, as required.
ADA Essential Functions Job Qualification Associate of Arts/Science Degree or equivalent college coursework, in Behavioral and/or Social Sciences, required.
Demonstrated interest in the field of Social Welfare via educational, previous employment, fieldwork, volunteer work, and/or life experience, required.
